McIntosh County 4-H Record Books
Each member applying for a county medal must fill out a County Record Book. In order to do that you must fill out the County 4-H Record Book Form & the Project Sheet for the medals for which the member is applying. If there is not a project sheet for the medal you are applying for please contact the Extension Office for an alternate form. Record Book forms must be turned in inside a folder. Official Record Book Folders are available at the Extension Office at any time for $2.00 if you would like to use the official folder.
